CONTEST RULES
ROGERS PUBLISHING LIMITED
1. Contest starts at 12:01 am(EST)on February 21, 2006, and continues until 5:00 pm(EST)on June 1, 2006, the Contest Closing Date. Entry for the Alaskan cruise Early Bird Draw continues until 5:00 pm on March 28, 2006(the Early Bird Draw Date). If you win the Early Bird cruise prize, you are not eligible for the Fall Cruise draw. No Purchase Necessary.
(a)To enter, fill out the entry form found in selected The Medical Post(tm)or Patient Care(tm)publications, and fax it to 416-764-3941, or mail it to CME Sea Courses Contest, One Mount Pleasant Road, 12th Fl., Toronto, ON, M4Y 2Y5 by the Contest Closing Date.
(b)Random draws will be made at approximately 10:00 am(EST)by a Rogers Publishing Limited representative("Rogers")at One Mount Pleasant Road in Toronto, Ontario to award prizes as follows:(i)on March 30, 2006 from all entries received by the Early Bird Draw Date;(ii)on June 6 from all entries received by the Contest Closing Date other than the winning entry from the Early Bird Draw Date.
(c)If a selected entrant cannot be reached within 5 days following the draw, incorrectly answers the skill-testing question, declines the prize for any reason, or fails to return a Release of Liability form as specified below, another entrant will be selected.
2.(a)Participants are eligible to win one(1)of the following CME Sea Courses cruise package prizes, each having an approximate retail value of $3,856.ea and consisting of the following:
(i)Early Bird Prize: One(1)of two Alaskan CME sea cruise for two(2)people aboard The Infinity(Celebrity Cruises), leaving Vancouver, British Columbia on July 16, 2006 and returning July 23, 2006, plus conference tuition for one(1)person.
(ii)Grand Prizes: One(1)of five(5)Caribbean Sea Courses cruise packages, each for two(2)people aboard the new Norwegian Jewel , leaving Miami, Florida on November 5, 2006 and returning November 12, 2006, plus conference tuition for one(1)person. Because of the nature of this prize, no other travel dates are available: winners must accept the prizes exactly as described.
Return flights to the specified point of departure plus all ground transportation and any associated accommodation costs other than on the cruise are the responsibility of the winners. Winners and travel companions are also solely responsible for all insurances(including medical), taxes, transfers, travel insurance, spending money, tips, meals and other travel and hotel charges not specifically mentioned as part of the prize; for making their own travel arrangements; and for obtaining their own travel documents(such as passport or travel visa)plus insurances in advance of the departure date. Rogers and Sea Courses are not responsible in the event winners and/or travel companions are refused entry to destination country or re-entry to Canada for whatever reason and any unused portion of the prize will be forfeit. Winner and travel companion must travel together on the same itinerary.
(b)Prizes must be accepted as awarded and may not be transferred or exchanged. Rogers and Sea Courses reserve the right to substitute a prize of at least equal value in the event of the unavailability, for whatever reason, of the advertised prize(s). Rogers and Sea Courses shall not be responsible in the event of cancellation or early termination of the cruise as advertised for any reasons beyond their control.
3. To be declared a winner, selected entrants must first correctly answer, unaided, a mathematical skill-testing question; and will also need to return a signed Release of Liability and Acknowledgment of Compliance with these Rules. Travel companions must also sign and return a Release form. Contest judges' rulings are final and without appeal in all matters related to the promotion and the awarding of prizes.
4. By entering this contest, entrants consent to the use of their entry, name, city of residence and/or any photograph of or that may be taken in publicity carried out by Rogers, Sea Courses and/or their advertising agencies, without further notice or compensation. Travel companions must also sign and return a Consent form for the taking and/or use of any photograph of or publicity carried out by Rogers and/or Sea Courses.
5. This contest is open to physicians who are residents of Canada and over the age of majority in their province or territory of residence, excluding residents of Québec and employees of Rogers and Sea Courses, its/their parent and affiliated companies, representatives, dealers and agents, contest suppliers and judges, and those with whom they are domiciled. The contest is subject to all applicable federal, provincial and municipal laws and regulations.
6. No communications will be entered into except with selected entrants. To receive a copy of the list of the winners, send a postage-paid, self-addressed envelope on or after the Contest Closing Date to The Medical Post, One Mount Pleasant, 12th floor, Toronto, Ont., M4Y 2Y5 Attn.: "Sea Courses Contest" or visit www.medicalpost.com after the Contest Closing Date.
7. Odds of winning depend on the number of eligible entries. All entries that are incomplete, illegible, damaged, irregular, have been submitted through illicit means, or do not conform to or satisfy any condition of the rules may be disqualified by the contest sponsor(s). Sea Courses and Rogers take no responsibility for lost, stolen, delayed, damaged, misdirected, late or destroyed entries, or for typographical or other production errors. Sea Courses and Rogers are not responsible for any errors or omissions in printing or advertising this contest. All entries become the property of the contest sponsor(s)and will not be returned.
8. Rogers and Sea Courses are collecting personal data about entrants for the purpose of administering this Contest. Entrants not wishing to receive any other informational or marketing communications from contest sponsor(s)should so indicate on the entry form, or contact us in accordance with Rogers' Privacy Policy. Please see Rogers' Privacy Policy at www.rogers.com for information on its policy towards maintaining the privacy and security of user information.
9. By entering this Contest, entrants release and hold harmless the contest sponsor(s), its/their advertising and promotional agencies and the contest judge(s), their affiliates and respective directors, officers, owners, partners, employees, agents, dealers, representatives, successors and assigns(collectively the "Releasees")from any liability in connection with this Contest or, if declared a winner, the prize(s). Before being declared a winner, entrants may be required to sign and return, within a stipulated period of time, a Declaration of Compliance with the Contest Rules and a full Liability and Publicity Release. Trip winners' guests must also sign and return a Declaration and a Liability/Publicity Release prior to being ticketed or within such other period as sponsor(s)may specify. Name of trip winners' guests cannot be changed after the Declaration and Release are received. By accepting a prize, winners consent to the use of their name, place of residence, voice, statements, photographs, or other likenesses for publicity, advertising or informational purposes in any medium or format without further compensation or notice.
10. This contest will be run in accordance with these rules, subject to amendment by Rogers and Sea Courses. Contestants must comply with these rules, and will be deemed to have received and understood the rules by participating in the contest. The terms of this contest, as set out in these rules, are not subject to amendment or counter-offer, except as set out herein.
11.(a)Rogers and Sea Courses assumes no responsibility for failure of telephone lines, the internet or websites during the promotional period, for any problems or technical malfunction of any telephone network or lines, computer on-line systems, servers, access providers, computer equipment, software, failure of any email or traffic congestion on the internet or at any website, or any combination thereof including any injury or damage to an entrant's or any other person's computer related to or resulting from playing or downloading any material in the promotion. Rogers and Sea Courses reserve the right to cancel or suspend the email portion of this contest should a virus, bug or other cause beyond their reasonable control corrupt the security or proper administration of the contest. Any attempt to deliberately damage any web site or to undermine the legitimate operation of this promotion is a violation of criminal and civil laws. Should such an attempt be made, contest sponsor(s)reserves the right to seek remedies and damages to the fullest extent permitted by law, including criminal prosecution.
(b)If the identity of an entrant is disputed, the authorized account holder of the e-mail address submitted at the time of entry will be deemed to be the entrant. The individual assigned to the e-mail address for the domain associated with the submitted e-mail address is considered the authorized account holder. A selected entrant may be required to provide proof that he/she is the authorized account holder of the e-mail address associated with the selected entry. All entries must be submitted from a valid e-mail account that may be identified by reverse domain name search. The sole determinant of time for the purposes of receipt of a valid entry in this contest will be the contest server machine(s).
